Appendix GG –Dermpath Slide Consult Protocol (Outpatient)

Outpatient

When to use:

-For patients with a prior biopsy read by an outside laboratory who may benefit from a ‘second opinion’

-Before a dermpath slide consult, always check the lab tab in EPIC for dermatopathology results as many outside Dermatology practices send their pathology to SLU Dermpath

If the case was read externally, in order to have our pathologists look at the slides (via a Dermpath consult), you will need:

  1. To get the path report from the outside lab: (identifies the external lab where the case was read and responsible for housing the slides):
  • If not in media tab or brought by patient, complete a release of information (ROI) form with the patient’s signature to obtain the path report from the outside Dermatologist who performed the biopsy (ROI forms are in the top counter drawer of patient rooms at CSM and at the nursing station at DP)
  • If you have the actual path report, you should print it out and proceed to step #2
  1. To get the actual tissue block sent to SLU Dermpath: Do step #1 AND fill out a second ROI to release the slides/block from the external pathology lab
  2. Dermpath order in EPIC:
  • In the gen derm visit encounter or orders only encounter under the gen derm context, search for ‘Dermpath Slide Consult’
  • Specify “lab collect”
  • Be sure the resulting agency is “Dermatopathology Laboratory” with the SLUCare address (ID 502)
  • Choose slides only or the tissue block for specimen source (the block is needed to perform additional stains)
  • Complete the lab location if available from the outside path report
  • If outside lab name not available, fill in the remaining information, and write ‘Lab/location pending outside records’
  • This information will need to be written in by hand by clinical support staff once the path report is received
  • Fill in the history and impression like a regular internal dermpath order (Figure 1)
  • Once signed, the requisition should print like a regular path order
  1. Hand the printed outside pathology report (if available), ROIs (1 or 2 forms depending on above), and Dermpath slide consult printed requisition to a specific MA. Document in your note which MA you gave it to as is done with biologics forms.
  2. The MA will scan the ROI and outside pathology report into EPIC Media
  3. They will hold the ROI and printed dermpath slide consult requisition until the pathology report is available, then upon receipt of the path report, will handwrite the name of the outside laboratory in possession of the slides/block onto the dermpath order req, staple the documents together and submit to dermpath in a bag along with the regular dermpath specimens
  4. Clinical support staff places record of order in biopsy book
