Resident/Fellow Time Away Request Form

This form must be filled out with at least 60 days notice of any changes to time away. Any less notice may result in not being approved. In addition, all clinics must be closed with more than 60 day notice and if requests include clinic modifications within that timeframe, they will be denied.
