Appendix V – Universal Biopsy Procedure

At the time of a biopsy:

1) Provider calls for nursing staff assistance.

2) Site of biopsy should be cleaned with alcohol and marked by provider.

3) Provider takes photos. Date of birth is verified with patient on Haiku and patient label.

4) Biopsy site is written on label and placed on specimen jar prior to biopsy being performed.

5) Dermpath requisition is filled out by provider WHILE nursing staff set ups biopsy, prints the labels and numbs patient.

6) Labels are double checked by provider to make sure the sites match those on requisition.

7) Biopsy performed by provider with ASSISTANCE from nursing staff.

8) Provider completes AVS WHILE assistant bandages site.

9) Nursing staff does not leave the room until bandage is placed.
