Grand Rounds

Patients with unusual cutaneous findings or management challenges can be invited to come to grand rounds, held the first and third Thursday mornings of each month.  Patients may present a diagnostic dilemma, therapeutic challenge, or a classic picture of an uncommon entity.  All residents, rotating students, full-time faculty and clinical faculty from the local community attend the conference.  Grand rounds starts with the examination of patients in the CSM clinic from 8:00 AM – 8:30, followed by discussion of cases in the Tiffany Conference Room from 8:30 – 9:30 am. Video conferencing of the discussion portion is available for community dermatologists to attend. The conference is run by Dermatology faculty on a rotating basis.  The second year residents are responsible for making sure that there are a minimum of three patients scheduled.  A Grand Rounds folder on OneDrive keeps track of scheduled patients as well as previous presentations.

Photo-only cases should be kept as backup in the event that a live patient does not show up or if a patient is unable to attend in-person.  The second years are in charge of running the conference:  getting slides to the dermpath fellow(s), collecting clinic notes and test results, calling patients to confirm, taking pictures on day of conference and making sure that discussion from conference is relayed to the patient and documenting this in Epic as a documentation only encounter or communicating with outside dermatologists.  The last resident to see the patient is expected to give a history during Grand Rounds (second years are to make sure that the chart info gets to the appropriate resident(s). Slide requests from the dermpath staff should include as much information as possible (i.e., name, case number, name of outside lab if not an inside case).  This request should be made by Monday morning the week of Grand Rounds so that they are available for photo taking by the dermatopathology fellow.  If patients are added on after this, the PCCs are to page the second year resident and the resident should get the name immediately to the dermpath staff.  This resident should return slides to the dermpath staff to re-file them or to mail them to the appropriate laboratory.  If the patient has pathology that is read elsewhere, the institution where the pathology was read is welcome to come present their pathology. In this circumstance it is the outside institution’s responsibility to take photos of the case.    In the event that the outside institution is not available to present their slides, at their request the slides can be sent to our dermatopathology laboratory and photos can be taken.

The dermatology residents may also request slides to be used for grand rounds purposes only; however, this option should be used sparingly with consideration of all circumstances surrounding the case and requires the resident discussing this with the dermatopathology attendings. In both of these circumstances, the dermatopathology faculty should be made aware of the case, including the name of the attending seeing the patient, by the dermatopathology fellow.  Second year residents will review follow up cases from previous Grand Rounds. If a resident sees a patient that was presented at Grand Rounds they should send the note to the second years to present as follow up at the next Grand Rounds.
